On receipt of a referral, you will be sent a
short pre-assessment questionnaire to complete and return to ensure
the individual concerned meets our general admission criteria.
You will then be contacted by the appropriate Manager to arrange a
mutually convenient date for a full assessment to be conducted at
the individual’s current place of residence.
Our Medical Director and Managers are happy to arrange visits to our
services for referrers, potential patients, their relatives, carers
and advocates.
